As an aside, why is it never a foul when the ball is running out and the defender makes no attempt to play it at all and holds the attacker off? Must be a foul, why are they never given?

The ball has to be withing playing (touching) distance of the defending player and the ref has to be able to decide that the defending player has control of the ball...

Exactly, it happens at least half a dozen times per game, the defender kind of either shields the ball to stop the attacker getting it or actually leans into the attacker to push him away from it.
