TV Tropes tells us what QI did. That in film it doesn't happen to a damsel 'for real' (i.e. she's not really in distress) and that it's only ever 'played for laughs'.  The one example 'for realsies' is a male being rescued. I appreciate Sidcow said 'anybody'.

I didn't know it was common in theatre though, what with me being an uncultured swine.

I thought we were talking about it actually happening IRL :D 

But yes, the theatrical / film trope happened before the IRL grim deaths

Yeah me and four friends were once, as luck would have it there was a utilitarian standing next to to the points lever. Bloke on the other line though.😔

This is quite a morbid post, but I saw an interview the other day of a woman who had basically had to live that thought experiment. 
 

She was driving down a road in Tasmania and there was a family crossing. Woman and 4 children. 
Just as she got to them one of the young boys ran out in front of her. 
 

She said she had time to make the decision of running over the boy or swerving and running over the family but had literally nowhere else to go. 

She ran over the boy
